Financial Policy

By purchasing through our store you are agreeing to our financial policies.

Thank you for choosing Silicon Valley Institute for Aesthetics (SVIA). Our staff is committed to assisting you in your care. 

I have chosen to purchase an elective cosmetic procedure and/or product with SVIA and understand and agree to the following: 

Online Payment.  All payments made online are final.  

Scheduling. Our staff will reach out to you within 24-48 business hours or your purchase to schedule your appointment or arrange product pickup.  Full payment of the treatment package is due at the time of the first treatment. 

Rescheduling. Patients who need to reschedule after their treatment has been booked can do so without penalty if SVIA is notified > 48 hours. There is a $100 administrative fee for rescheduling your treatment if SVIA is notified < 48 hours. 

Refunds. All payments made online are final.  Deposits can be used as credit in our practice towards another treatment or product.

Office Preference.  Please note, the location you choose at checkout is the location you must have your procedure.

Waiting Rights. Under the Health Insurance Portability and Accountability Act of 1996 (HIPAA) guidelines, you will comply and waive your privacy rights in the event of a dispute post-surgery or treatment.  As parties may need to be consulted and involved outside of our practice, we must be able to distribute documented and protected information to reach a resolution.

Full payment of services is required on the date of the procedure. SVIA accepts cash, cashier’s checks, and all major debit and credit cards. Credits cards must be present at the time of charge. 

With any non-surgical procedures there are risks and complications that may result in additional procedures, lab tests, consultations, medications etc.  Patients are responsible for any additional fees incurred.
